Service Description
Ishihara: Color Blind Test is a quick and reliable screening used to detect red-green color vision deficiencies. How it works: This test uses a series of plates with colored dot patterns that form numbers or shapes, which individuals with normal vision can identify. Snellen: Standard visual acuity test using the Snellen eye chart to measure clarity of distance vision. Commonly required for DOT physicals, pre-employment screenings, school, and general wellness exams. How it works: The client reads letters of decreasing size from a standardized chart at a set distance. Results are recorded as a visual acuity score (example: 20/20, 20/40). Jaeger: A near vision acuity test using the Jaeger chart to assess the ability to read small print at a close distance. Commonly required for pre-employment screenings, DOT physicals, and positions where fine detail work is involved (such as technicians, assembly, or inspection roles). How it works: The client reads progressively smaller lines of text or symbols from a Jaeger chart held at a standard reading distance. Results determine near vision clarity. Titmus: A comprehensive vision screening performed using a Titmus Vision Screener device. Assesses distance vision, near vision, depth perception, peripheral vision, and color vision — all in one test. Widely used for DOT physicals, pre-employment screenings, and occupational health requirements. How it works: The client looks into the Titmus machine and completes a series of automated vision tests. Results are recorded and provided for employer documentation or personal records.
Cancellation Policy
*Privacy & Confidentiality Thrive Screening Solutions is committed to protecting client information in compliance with HIPAA and state privacy laws. All collected health data, test results, and personal information are securely stored and only shared with authorized individuals. Clients must sign a consent form before testing, and results are delivered through secure channels only. *Booking & Appointments Appointments may be booked up to 30 days in advance and as late as 1 hour before start time, based on availability. Same-day bookings are available for most services. DNA Testing, DOT Physicals, and Occupational Health may require a 24-hour notice — please contact us. *Rescheduling & Cancellations Appointments may be rescheduled or canceled up to 24 hours before the start time. For mobile services, cancellations within 12 hours may incur a dispatch fee. Missed or late-canceled appointments may be subject to a fee. *Refunds Refunds are available for prepaid services canceled within the approved cancellation window. No refunds will be issued once testing has been performed or samples have been collected. *Identification & Chain of Custody All clients must present a valid government-issued ID prior to any testing. Drug and DNA tests requiring legal documentation follow a strict chain of custody process with full documentation and secure handling of all specimens. *Results & Reporting Rapid test results: available within 10–30 minutes. Lab-based results: returned within 1–5 business days, depending on test type. All results are reviewed and securely shared with the client and, if applicable, with authorized employers or agencies. *Mobile Testing & Fees Mobile or on-site setup fees may apply. Mileage fees apply beyond a 15-mile radius. Clients must provide a clean, private area for testing. *Post-Accident & After-Hours 24/7 post-accident & after-hours testing is available by appointment. Additional fees apply. Clients must provide accurate location details. *Refusal to Test We reserve the right to refuse service if a client is uncooperative, fails to provide proper identification, or disrupts the testing process. In such cases, applicable service fees will still apply.
